 Job Title: Pharmacist
Location: NEAR Hartford Connecticut

Reports to: Director of Pharmacy




·         The Registered Pharmacist is responsible for the provision of pharmaceutical care to all patient age groups (including geriatric, adult, adolescent, pediatric and neonatal patients). 

·         They are responsible for the preparation, dispensing and monitoring of both the prescribing and effectiveness of all medications and IV admixtures. 

·         They exercise considerable independent decision making.



·         Completion of course at an accredited School of Pharmacy, with a degree of Bachelor of Science in Pharmacy or Doctorate of Pharmacy. 

·         Experience:  Registration in Connecticut (or reciprocal State) as a Pharmacist. 

·         LTC Pharmacy experience desirable. 

·         Familiarly with Federal and State laws regulating professional and LTC practice of Pharmacy. 

·         Understanding of role of LTC Pharmacy, its interrelationships with nursing facilities and functions. 

·         Successful completion of all orientations




·      Utilizes the medication screening process to insure medication orders are complete, appropriate, within legal parameters and prioritized

·      Supervises technical staff and volunteers to ensure accuracy and compliance with policies and procedures

·      Oversees dispensing, and compounding of medications

·      Willingness and flexibility to work alternating and rotating work schedules on short notice

·      Displays self initiative in anticipating and resolving daily work issues

·      Evaluates and reports Adverse Drug Reactions

·      Participates in on-going education and training related to organization and department specific requirements

·      Notifies nurse of any changes/modifications of medications prescribed.

·      Follows proper safety and infection control procedures and maintains a safe work environment.  Participates in the safety and infection control management programs

·      Assumes role of drug information provider to all members of the health care team specific to patient populations and age groups

·      Provides direct supervision of all non-pharmacist staff personnel

·      Monitors specific medication prescribing in accordance with established drug usage criteria, prior to dispensing, as defined by the Pharmacy & Therapeutics Committee
