Qualifications
• Bachelor’s Degree in Physical Therapy
• 2+ year’s experience in physical therapy
• CPR Certification
• California Physical Therapy License
• Valid CA Driver’s license
• Continuous automobile insurance coverage

Primary Duties
- Evaluate referred patients and develop treatment plans including specific goals and activities, reevaluating and modifying plans as necessary.
- Provide direct physical therapy to patients per plan of care.
- Provide instructions in therapeutic procedures to be continued by the patient.
- Maintain active and support communication with the patient/caregiver regarding progress, problems, home programs and other issues related to the therapeutic process.
- Prepare written documentation as required by the profession and the department such as evaluation results, individualized treatment plans and progress reports.
- Communicate observations, facts and comments with the interdisciplinary team.
- Attend patient care conferences and other related meetings.
- Assist in the Quality Improvement Program and in providing inservices to other staff.
- Enhance professional growth and development through participation in education programs, current literature, workshops and inservice meetings.
- Demonstrate sensitivity to the needs of all cultural and age groups, predominantly the geriatric and dying patient.
Qualifications
• Bachelor’s Degree in Physical Therapy
• 2+ year’s experience in physical therapy

• Valid CA Driver’s license
